Promoting the health and dignity of frail elders

Many elders struggle with disability and severe chronic illness and have difficulty getting to the doctor’s office. As a result, their health may suffer and lead to unnecessary ER visits, hospitalization, or nursing home care. In 1999, recognizing the needs of such elders and their families, we created the MedStar House Call program - MedStar Total Elder Care to provide full medical and social services that help elders remain in their homes with dignity. These teams now serve both Washington, D.C., and Baltimore, MD.

The MedStar House Call Program - MedStar Total Elder Care is nationally recognized for the quality and outcomes of our care. We serve patients at home with a team of geriatricians, nurse practitioners (NPs), social workers, office nurses, and coordinators. We make routine and urgent house calls (in-person or via telehealth with video or audio-only phone visits). Our medical staff is also available by phone 24/7 for urgent issues. We provide access to state-of-the-art hospital and specialty care. Additionally, our physician team follows our patients if and when they are admitted to the MedStar Washington Hospital Center.

Hospital care and specialists

Our team coordinates emergency, hospital, and specialty care at MedStar Washington and MedStar Good Samaritan. Patients may keep their previous specialist doctors; we work with them as needed. We refer to MedStar Good Samaritan, MedStar Washington, for home-based podiatry or other new specialist care. If House Call patients require hospitalization, and 911 is not needed, our team can arrange transportation and admission to MedStar Good Samaritan or MedStar Washington when beds are available.

Psychosocial assessment

Care Coordination

Development of an individual treatment plan

Information and referral to community resources and supports

Caregiver education, support, and counseling

Advocacy to connect with other service networks and legal counseling

Crisis intervention

Assistance with identifying alternative living arrangements, as needed

Enrollment and insurance.

To qualify for the MedStar House Call Program, patients must:

Be 65 years or older and have difficulty getting to the doctor’s office

Have Medicare, Medicaid, or another participating insurance plan

Stop seeing their previous primary doctor and agree to have us take on that role following the first visit

Live in a qualifying ZIP code

Our office is open 8:30 a.m. to 4:30 p.m., Monday - Friday.

When calling about new patient enrollment, please have the following information:

Patient's name, address, telephone number

Patient's date of birth and social security number

Patient's next of kin and emergency contact information

Patient's health insurance information (including type and group number)

Patient's current medical conditions and concerns

Ability to retrieve patient's recent medical records

As in most doctor's offices, your main health insurance (such as Medicare) covers 80 percent of House Call visit fees, and any secondary insurance covers the other 20 percent. The patients who do not have any secondary insurance are responsible for the 20 percent co-pay.

Getting your older adult to the doctor’s office for an appointment can be difficult or sometimes impossible.

Whether they’re frail, can’t walk on their own, or have Alzheimer’s or dementia , getting out of the house is hard on both of you. Going to a doctor’s office can also expose seniors to germs or harsh weather.

We explain what a house call doctor is, what type of insurance they accept, how to find one, and what to look for in terms of services.

What is a house call doctor?

Today, many doctors are bringing back the old practice of visiting patients in their homes. With house calls, older adults don’t have to go through the stress and difficulty of getting to the doctor’s office. And neither do you.

Another bonus is that these doctors usually spend more time with patients. You won’t have to fit all your questions into a 15 minute visit.

Do they accept Medicare?

Yes, many house call doctors accept Medicare , private insurance, and sometimes Medicaid . It usually costs the same as a regular office visit.

But every house call doctor is different, so make sure you understand their fees and accepted insurance plans before making an appointment.

How to find a house call doctor

Some large health care systems like Kaiser Permanente or the VA have programs that include home visits by doctors and nurses. For example, Kaiser’s home-based palliative care program includes house calls.

Even your older adult isn’t part of a large health network, it’s worthwhile to ask your older adult’s doctor if they have home doctor visit programs.

The American Academy of Home Care Medicine’s provider directory is another way to locate a house call doctor in your area.

You can also use Google to search for “house call doctor” + your city or county or “home doctor visit” + your city or county (don’t include the quotation marks).

What to look for in a house call doctor

Before booking an appointment, make sure you understand the doctor’s services, fees, and billing.

Questions to ask:

  • Do you specialize in treating seniors, people with Alzheimer’s or dementia , or those with  multiple chronic conditions ?
  • Do you accept Medicare , Medicaid , or my older adult’s private insurance plan?
  • If we want, can we also keep my older adult’s primary care doctor?
  • Will you communicate with my older adult’s existing doctors and specialists so their care is coordinated?

Patients with difficulty leaving home due to a mobility limitation, patients with difficulty leaving the home due to cognitive impairment, patients who require the use of special transportation to leave the home, patients with an inability to leave the home and for whom leaving the home requires taxing effort, patients requiring the assistance of another person to leave the home, patients for whom leaving the home is medically contradictory due to the patient's medical condition, accepted insurance.

Did you know you can set up a yearly preventive care visit at home.

UnitedHealthcare® HouseCalls — our yearly in-home health and wellness service — is a health plan feature for Medicare and Medicaid members, offered at no additional cost to you. It’s a way to help our members stay on top of their health between regular doctor visits.

The goal of our preventive care visits is to help you maintain your independence and continue doing what you love long into the future. We know there’s no way to prevent all health emergencies and accidents, but we believe that by taking care of yourself, you may be able to improve your odds of avoiding them. That’s where HouseCalls comes in.

These days, you can have just about anything delivered to your door.

How about a home delivery of good health and wellness? It comes from UnitedHealthcare and is called HouseCalls.

HouseCalls is a yearly health and wellness visit that happens in the privacy of your very own home. No driving to an appointment. No sitting in a waiting room.

Here’s how it works.

Our HouseCalls team finds a time that’s convenient for you and schedules the visit. A health care practitioner, a physician’s assistant, medical doctor or nurse practitioner, comes to you at the scheduled time. The clinician completes a health evaluation, including a physical exam and other important screenings, then YOU decide what happens next.

You can discuss questions to ask your doctor at your next appointment, review instructions from your last doctor’s appointment or chat about other health concerns you haven’t had a chance to talk to your doctor about. Whatever you need.

The visit takes about 45-minutes to an hour – plenty of time to answer your important health questions.

Your practitioner will even send a summary of your HouseCalls visit to your doctor.

And unlike those other home deliveries – there’s no additional cost for a HouseCalls visit, it’s a feature of your medical plan. If you’re in good health or you already see your doctor regularly – you might not think HouseCalls is for you, but it is. Think of it as an extra layer of care – valuable one-on-one time you don’t always get in the doctor’s office.

So, when you get a phone call from our HouseCalls team – take advantage of this great program and schedule your HouseCalls visit. Or just call us and make an appointment!

What to expect during a HouseCalls visit

During a HouseCalls visit, you’ll meet with the same types of professionals you’d see in a doctor’s office — a licensed physician or nurse practitioner. They’ll answer your health-related questions, perform a physical exam and offer a health screening in the comfort of your own home. If you choose, your family members, loved ones or caregivers are welcome to sit in on this visit and ask additional questions. A typical visit will range from 45 minutes to a full hour.

The results of your exam and screening will be forwarded to your primary care provider (PCP) and you’ll receive a summary of your visit in the mail. 

During your visit, here are some typical topics you'll cover

  • An overview of your medical history and prescription medications
  • Conversations about overall well-being, like how to help you stay active, sleep well and manage stress
  • Tips on to help you avoid trip hazards in your home
  • Community resources and support

Open the door to the convenience of a medical visit in your home

Meet with licensed medical staff on your schedule, in the convenience of your home. Once they arrive, talk about health concerns at your pace (the visits are focused on you and you alone). Use the results to help coordinate care with your doctor. 

  • Be prepared to provide a urine sample during your visit—drink fluids in advance
  • Wear shoes or slippers that are easy to remove (your feet will be checked)
  • Know where to find medication bottles so they are ready to review 
  • If applicable, have blood pressure readings available for review
  • If you have diabetes, please provide blood sugar test results, as well as your blood glucose meter 

Remember, at the end of your visit, you’ll receive a personalized checklist of topics to discuss with your doctor at your next appointment. Plus, you and your doctor will receive a mailed summary of your visit.

The UI Health Physician Home Visits Program, staffed by the excellent physicians from the Family Medicine Clinic , brings compassionate, comprehensive medical care to the homes of patients of all ages, enabling them to

  • Were recently discharged from the hospital, Emergency Room, or Critical Decision Unit

Making an Appointment An initial house call can be scheduled by contacting our office at 312-996-1119 .

New patients must complete registration and have a brief eligibility screening by phone. The physician home visit will be scheduled within a week of contacting our office.

Follow-up Visits Follow-up visits will be scheduled during each home visit as needed. If a problem develops before the next scheduled visit, the patient or caregiver can contact our office and schedule an earlier appointment.

Refills Medications are refilled during regular office hours by calling our office at 312.996.2901 . Leave a message for the nurse if necessary and she will call you back within 24 hours. Please allow 48 hours for all refill requests.

Billing and Insurance The UI Health Home Visits Program accept Medicare, Medicaid, and most major insurance providers.

Home Visit Physicians' Hours Visits are scheduled as needed, and will usually occur between 7 am and 5 pm. Some home visits may occur on Saturday. To schedule a house call, please call 312-996-1119 .

How to Contact us after Business Hours Although our family doctors do not make late evening or night visits, we provide a 24-hour answering service to reach a physician for a consultation or to determine if urgent or emergency care is needed. Call our office at 312.996.2901 , and a physician will return your call, usually within one hour.

Mobile Physician Services (MPS) is a practice of doctors, nurse practitioners, and physician assistants who make house calls to homebound patients. We specialize in the treatment of patients with complex medical needs.

MPS is one of the only house call practices in the United States that has both primary care and specialist providers on staff. Our team of professionals enables us to deliver convenient, comprehensive services to treat the whole patient.

MPS was established in 2006 to fill an unmet need for medical care among patients who were unable to leave their homes or who had difficulty getting to a physician’s office. Often these patients had complex chronic conditions that required ongoing medical care. Without proper care, their condition would worsen, and the patients eventually would require emergency care or admission to a hospital – at a high cost to both patients and payers.

At MPS our approach centers around providing ongoing primary care with specialty services as needed. This enables us to monitor patients’ conditions and address problems before they become urgent, reducing the need for emergency care and hospital admission and improving patients’ quality of life.

Headquartered in New Port Richey, Florida, our practice continues to grow each year. In addition to providing in-home services, we also work with assisted living facilities and hospitals to provide ongoing or post-procedure care.

Hi, my name is Nancy Baton. I am a retired high school principal and middle school principal. And I am truly enjoying my retirement.

I live with my husband, and I’ve been married to Bill for, it’ll be 38 years this summer. We have two children, grown children, and I have 5 grandsons.

When it came time for me to enroll in Medicare, I wanted the one with the least amount of hassles. And I had been told what a great health plan Aetna has. That’s why I chose Aetna and I’m very happy I did.

Every year Aetna offers the opportunity for a Healthy Home Visit. The nice part about the HHV is I don’t go anywhere. The nurse comes here to me. So I’m comfortable in my own environment. I don’t have to leave the house. And It’s just nice to have somebody come to you for a change.

It was a registered nurse and she spent about an hour and a half. Goes over my medications. Checks my blood pressure. And then said to me she thought I have a problem with circulation. So she did a test on me and my doctor called me and said you need to see a vascular surgeon. And I’m under his care. Now I never would have caught something that could be potentially very dangerous had that nurse not come to my home.

Having the support of Aetna and not having to worry about bills or paperwork or is this covered. I didn’t have any of that. All I do now is concentrate on my recovery.

If you’re not healthy, you’re not going anywhere. So Aetna has helped me stay on track.

Before your visit

Once you schedule your Healthy Home Visit through Signify Health, you’ll get a confirmation for your appointment. You’ll also get an email, text or call reminder 24 hours before your visit. If we don’t hear from you to set up your appointment, a member of the Signify Health team will reach out to help schedule your visit.

During your visit

On the day of your visit, a nurse will spend up to an hour with you in your home. They’ll answer health questions, address any concerns and more. This includes:

  • Checking vital signs and reflexes
  • Reviewing medical history
  • Going over medications
  • Checking breathing, eyes or feet
  • Perform preventive screenings 

After your visit

A summary of the visit will be shared directly with you and your doctor. Should anything with immediate concern come up, we help coordinate the care you need.

Primary Care Doctors

Primary care doctors are the entry point to the medical system. They provide a comprehensive approach to care, including prevention, treatment and rehabilitation of health issues. Primary care doctors also help coordinate care when you need to see a specialist.

Having a primary care doctor allows for an ongoing patient-doctor relationship built on mutual trust.

There are different types of primary care doctors :

  • Internal medicine doctors. Also called an internist , internal medicine doctors manage common and complex illnesses typically for adults and older adults.
  • Family medicine doctors. Family medicine doctors provide continuous care for people of all ages, backgrounds and conditions, often caring for entire families from birth through the end of life.
  • Pediatricians. Pediatricians are a type of primary care doctor specializing in the care of infants, children and adolescents. They provide primary and preventive care, including administering routine immunizations or providing acute care for common childhood illnesses , like ear infections, colds and flu and minor injuries.

While starting with a visit to your primary care physician is a great way to assess whether or not you need to see a specialist, it's also important to schedule return visits so that chronic issues can be monitored and new ones addressed.

“Sometimes, I’ll say to the patient, ‘I can take care of that. You don’t need to go to the specialist at this point in time,’” says Dr. Theodore Strange, chair of medicine at Staten Island University Hospital, a division of Northwell Health, in New York City.

In fact, upwards of 95% of cases can be taken care of with good primary care , says Dr. David Cutler, a board-certified family medicine physician at Providence Saint John’s Health Center in Santa Monica, California. “The referral itself is the exception,” he adds. Primary care physicians also ensure completion of routine surveillance, such as immunizations, mammograms and colonoscopies.

When to See a Medical Specialist

However, there are a few reasons doctors may refer patients to a specialist, including:

  • If the patient has a certain chronic or complex disease with symptoms that may need specialist testing. For example, someone with a cardiac condition may need a stress test, or an individual with a GI condition may need a specific type of endoscopy or colonoscopy.
  • If you have the results of testing and need a more expert interpretation of the test to better treat the case.
  • If there’s a patient with an uncertain diagnosis. A primary care physician may refer you to a specialist if they have run out of options and need an expert with more specialized medical knowledge to diagnose and treat you.
  • If you need or want a second opinion .

Types of Medical Specialists You Might Need

Medical specialists have additional training and expertise in a specific area of medicine. Some even subspecialize, meaning they have even more specialized training within their specific field of medicine.

Do You Need a Referral to See Specialists?

Whether or not you need a referral to see a specialist depends on your health insurance plan , your doctor or the type of specialist you’re trying to see.

Point of service, or POS, plans and health maintenance organizations, or HMOs, often require a primary care physician to work with your insurer for pre-approval to visit a specialist. The referral also likely needs to be for a specialist within your insurance network. If you’re unsure whether or not you need a referral, check with your insurance company first. Otherwise, your plan may not cover the care and you’ll get stuck with high out-of-pocket costs.

In addition to the patient’s health insurance coverage, symptoms , medical history and diagnostic testing and results, primary care physicians will likely take these factors into consideration when making a referral:

  • Accessibility . It's important for the specialist to be geographically accessible to the patient.
  • Availability . Some specialists can take months to make an appointment with, so referring doctors take their availability into consideration.
  • Coordination of care . When physicians are part of a broader health network, they’ll likely recommend a specialized doctor within the same network. One benefit is shared electronic medical records, which helps streamline follow-up coordination of care.

Covid cases are rising: When to rest and when to test, according to CDC and experts

Covid rates are still rising across most of the country, fueled by the highly contagious new variants of the virus —  KP.2 and KP.3 and LB.1. As of July 18, the highest levels of Covid are in the Western states, although there are no areas of the U.S. where rates are noticeably dropping, according to CDC wastewater data .

At least 21 states have very high levels of Covid , the data shows.

“You have a very, very transmissible variant, probably one of the most transmissible we’ve had,” said Dr. Peter Chin-Hong, a professor of medicine and infectious disease specialist at the University of California, San Francisco. “We have a lot of population immunity, but it doesn’t mean that there isn’t a lot of chaos.”

But people who test positive for Covid don’t need to isolate for five days, according to recent guidance from the Centers for Disease Control and Prevention.

This is the first major wave since the CDC updated its Covid guidance , said Katelyn Jetelina, an epidemiologist who tracks illnesses on her website, Your Local Epidemiologist . She recommends isolating until an at-home Covid test turns negative, which could be anywhere from three to 15 days.

Yet, with fewer people testing for Covid, experts say it’s more important than ever to pay attention to symptoms. Since March, the CDC’s guidance has matched advice for flu and other respiratory illnesses: Stay home when you’re sick, but you can return to work or school once you’re feeling better and you’ve been without fever for 24 hours. 

“The guidelines reflect a number of things, including the fact that we generally are overall shedding for less time now,” Bill Hanage, an associate director at the Center for Communicable Disease Dynamics at Harvard University, said. “And there’s more of a link between symptoms” and being contagious.

Because most Americans have already either had a version of the virus or have had several vaccines or boosters — or have had Covid and are also vaccinated —  it’s hard to separate Covid symptoms from other respiratory illnesses without testing. Right now, chances are, if you have upper respiratory symptoms — sniffles or congestion — or a cough, it could be Covid. 

According to the CDC, someone with Covid can go back to normal activities when symptoms have been getting better for at least 24 hours and no fever even without taking fever-reducing medication like acetaminophen or ibuprofen. 

Then for the next five days, take precautions such as wearing a mask in crowded indoor situations. 

The majority of viral spread happens when people are the sickest. 

“As the days go on, less virus spreads,” CDC Director Dr. Mandy Cohen said in an interview this year.

The CDC guidance is for the general public only, and does not include isolation guidelines in hospital settings, which is generally 10 days.

Covid symptoms

For people who have been through at least one Covid infection, or are up to date with the vaccines, symptoms often resemble a common cold or other respiratory illness.

Common Covid symptoms include:

  • Fatigue and muscle aches
  • Sore throat
  • Nausea or diarrhea

There's not really a major sign that indicates Covid, doctors say, so the only way to know for sure is to take a test.

When should I test for Covid? How often?

The original guidance from the CDC was to test approximately five days after exposure because it took that long before the virus became detectable in the body. Now, with high levels of prior exposure, some people could feel symptoms a day or two after exposure, not because the virus is growing more quickly but because the immune system recognizes the virus and responds symptomatically, Michael Mina, an epidemiologist and former professor of immunology and infectious diseases at the Harvard T.H. Chan School of Public Health, said.

While many people aren't bothering to test now — tests aren't free anymore — Mina advises taking an at-home test on the third day after symptoms begin. If it's negative, test again the next day.

"If I had only two tests, that's where I'd stop testing," Mina said. "If positive, you don't need more tests. My best educated guess is, assume you're infectious for five days after testing positive."

In a prior study, Mina said that up to 50% of people were still infectious a week or eight days after symptoms begin and up to 20% were still contagious up to nine days later.

While people's immunity is doing a better job than a year ago, Mina believes many people could still be contagious five days after symptoms begin.

The experts recognize that most people will be ready to get back to work or school quickly after they feel better. What's most important is to be careful around people who may be immunocompromised or elderly.

"You don’t want people to be locking themselves in their bedrooms for five days," said Hanage. That said, "there are some contexts where you really would want to be more careful, including giving it to somebody who’s vulnerable for whatever reason."

Online Rumors Baselessly Claim Biden Experienced Medical Emergency After COVID-19 Diagnosis

President Joe Biden tested positive for COVID-19 on July 17. His symptoms remained mild and have since resolved, according to his treating physician. But fueled by days without a public sighting and Biden’s decision to drop out of the presidential race, social media posts have baselessly claimed that Biden was gravely ill or had a medical emergency.

On July 21, President Joe Biden announced he was no longer running for president in the 2024 election. Just four days before, while campaigning in Las Vegas, he tested positive for COVID-19, leading him to fly to Delaware that evening to isolate. He remained in isolation at his beach house and was not seen in public again until July 23, when his symptoms had “resolved” and he tested negative for COVID-19, according to his physician.

Daily letters from Biden’s doctor show that the president never got seriously ill and he continued to perform his duties. But since his COVID-19 diagnosis — and particularly after his announcement that he would no longer seek the presidency — online posts have trafficked in unfounded rumors and speculation about Biden’s health.

“A verified source has informed the Global Press team that Joe Biden is currently in hospice care and is unlikely to survive the night,” proclaimed a July 22 X post later shared on Facebook. The @globalpresscorp account on X that made the original claim appeared to subsequently delete its account. (That account should not be confused with Global Press Journal, a legitimate international news organization that does not cover the U.S.)

Some declared Biden to be terminally ill or even spread rumors that the president was dead .

Others , including conservative commentator Charlie Kirk, pointed to purported reports from local police in Las Vegas to suggest or claim that Biden had or might have had a medical emergency. 

According to an unnamed source, Kirk said, “US Secret Service informed LV Metro that there was an emergency situation involving Joe Biden and to close necessary streets so that POTUS could be transported immediately to University Medical, which they began to do in earnest.” 

“Then, mysteriously, there was a stand down order and the USSS informed local Vegas PD that they were going to ‘medivac’ POTUS to Johns Hopkins, which they presumed meant fly him back east ASAP,” Kirk’s post continued. “Apparently the rumor mill in the police department was that Joe Biden was dying or possibly already dead.”

Kirk’s speculations were later amplified in a Tucker Carlson video on Instagram. Citing an unnamed, non-medical source, another person online claimed Biden might have had a transient ischemic attack, or a mini stroke. 

Former President Donald Trump also chimed in on July 21, baselessly saying on his social media platform, Truth Social, that Biden “never had Covid.”

It’s true, according to local media and a statement from the police department, that a hospital in Las Vegas was on standby to potentially receive the president after he tested positive for COVID-19, and that police proactively shut down roads to ensure safe passage. But this doesn’t mean Biden had a medical emergency. The president never went to the hospital, and according to a letter posted by his treating physician, he only ever experienced mild COVID-19 symptoms. In an email to FactCheck.org, the White House denied that Biden had any kind of medical emergency.

In the afternoon of July 23, Biden appeared on a tarmac and told reporters he was “feeling well” before ascending the stairs to Air Force One to fly back to the White House. This put to rest the most extreme claims about his health, although conspiracy theorist  Laura Loomer continued to insist he was dying. Biden is scheduled to address the nation this evening to discuss his decision to exit the 2024 presidential race.

No Evidence of a Medical Emergency

While suspicion has focused on road closures and preparation for a hospital in Las Vegas to possibly receive the president, there’s no evidence that Biden experienced a medical emergency. Instead, the posts appear to have spun contingency plans following Biden’s positive COVID-19 test into conspiracy theories.

“President Joe Biden did not visit UMC last week,” Scott Kerbs , a public relations director for University Medical Center of Southern Nevada, said in a statement to FactCheck.org. “On July 17 at approximately 2:15 p.m., UMC received notification to prepare for a potential medical visit from the president. As Nevada’s only Level I Trauma Center and the hospital tasked with caring for the Commander-in-Chief during official visits to the area, UMC was fully prepared to provide the state’s highest level of care for the president.”

In a local news report on July 17, the hospital’s CEO, Mason Van Houweling, indicated that the hospital prepared for a “medical” issue, rather than a trauma or accident, but did not elaborate further. He praised his hospital’s response, but referred to it as a “non-event.”

Van Houweling echoed that sentiment in another article about the hospital’s preparations, published on July 19 in Becker’s Hospital Review, which noted that the hospital “adjusted its preparation to arrange an appropriate facility and team for medical care based on the specific potential needs identified — in this case, diagnostic requirements.”

Kerbs told us in an email that this was describing “the fact that UMC, as a Level I Trauma Center, adjusted its default preparations from trauma care to general medical care, which involves diagnostics.”

Local police similarly confirmed that they prepared for Biden to travel to the hospital, but explained that it was precautionary. 

“The Las Vegas Metropolitan Police Department was notified that President Joe Biden was sick on July 17th during his visit to Las Vegas. We did not know the nature of his illness,” the department told us in an email. “As a precaution, LVMPD proactively began to shut down roads leading to UMC Hospital. The Secret Service then advised LVMPD the President was going directly to Harry Reid International Airport and would be leaving Las Vegas.”

“It has been standard practice for many years, across administrations, for hospitals to be among the wide range of resources that are always put on standby when any president travels,” Andrew Bates, a White House spokesperson, told us in an email.

Bates denied that the president experienced a medical emergency, including a mini stroke. “The only medical situation was his covid diagnosis, which was publicly announced,” he said. 

A reporter who accompanied the president on the flight back to Delaware did not make note of any kind of medical emergency. Instead, he reported that the flight was “quite bumpy,” and that his request for an informal press briefing was denied. Upon landing, while not looking in the best of health — as might be expected for someone sick with COVID-19 — Biden was still able to walk and speak.

COVID-19 can be dangerous , particularly for older adults and those who have not been infected or vaccinated previously, but all available evidence points to the 81-year-old president’s case as being mild. 

Dr. Kevin C. O’Connor, Biden’s physician, provided daily updates on the president’s condition. In his last letter, on July 23, he reported that Biden had tested negative for the coronavirus and his symptoms, which had included a runny nose, cough and general malaise, had “resolved.”

“Over the course of his infection, he never manifested a fever, and his vital signs remained normal, to include pulse oximetry. His lungs remained clear,” O’Connor said of Biden, adding that the president “continues to perform all of his presidential duties.”

Although Biden did not make a public appearance until July 23, he called into his former campaign’s headquarters — now Vice President Kamala Harris’s HQ — on July 22. While in isolation, he also received briefings , spoke to a variety of politicians and supporters following the termination of his candidacy, and made a call to the head of the European Commission, according to White House officials.

As with his first bout of COVID-19 in 2022, Biden took the antiviral Paxlovid to treat his illness.

It’s not the first time that social media claims about Biden’s health have gone viral. Earlier this month, popular posts falsely claimed Biden had a “medical emergency” aboard Air Force One, the AP reported .

Across the world, critical businesses and services including airlines, hospitals, train networks and TV stations, were disrupted on Friday by a global tech outage affecting Microsoft users.

In many countries, flights were grounded, workers could not get access to their systems and, in some cases, customers could not make card payments in stores. While some of the problems were resolved within hours, many businesses, websites and airlines continued to struggle to recover.

What happened?

A series of outages rippled across the globe as information displays, login systems and broadcasting networks went dark.

The problem affecting the majority of services was caused by a flawed update by CrowdStrike , an American cybersecurity firm, whose systems are intended to protect users from hackers. Microsoft said on Friday that it was aware of an issue affecting machines running “CrowdStrike Falcon.”

But Microsoft had also said there was an earlier outage affecting U.S. users of Azure, its cloud service system. Some users may have been affected by both. Even as CrowdStrike sent out a fix, some systems were still affected by midday in the United States as businesses needed to make manual updates to their systems to resolve the issue.

George Kurtz, the president and chief executive of CrowdStrike, said on Friday morning that it could take some time for some systems to recover.

What was affected?

It is more apt to ask what was not affected. Everything from airlines to banks to health care systems in many countries was hit.

In Australia, passengers were stuck in long lines at Sydney airport as information screens went blank, and programming was disrupted at the national broadcaster. Airports in Britain, Germany and Taiwan had long delays at check-ins and flights were delayed or canceled. At an airport in South Korea, handwritten boarding passes were being slowly handed out.

Flights continued to be disrupted at some U.S. airports into the morning because of the cascading effect of flight delays and cancellations. The Federal Aviation Administration said in a statement that ground stops and delays would be “intermittent” at some airports as airlines grapple with residual technology issues.

The outage affected emergency 911 lines in multiple states, the U.S. Emergency Alert System said on social media — but most if not all of the emergency system problems appeared to be resolving themselves by midmorning.

A few hospitals in Germany said they would cancel elective procedures; and in Britain, some doctors in the National Health Service were unable to gain access to systems. Kaiser Permanente, a medical system that provides care to 12.6 million members in the United States, said all of its hospitals’ systems were affected, and it activated backup systems to keep caring for patients.

At some banks, including JPMorgan Chase, there were delays in processing trades because bankers could not log into their work systems. TD Bank, the 10th largest in the United States, said customers complained that they could not access their online accounts.

But the problems were not uniform. London’s Heathrow Airport said that its flights were still operating. The London Stock Exchange said that it could not publish news updates but the exchange, where trades take place, was working as normal. The auction system at the Norwegian central bank was briefly interrupted, but other major central banks, the European Central Bank and Bank of England, said there was no effect on their systems.

In some cases, issues were resolved relatively quickly. In Ukraine, Sense Bank and the mobile operator Vodafone reported brief problems with their services. At Dubai International Airport, two airlines switched to alternative systems, allowing operations to resume.

Major grocery chains in the United States appeared largely unaffected, with most stores operating as usual. But the world’s biggest logistics companies, including United Parcel Service and FedEx, did report disruptions, causing delivery delays in some regions. A spokesman for UPS said the company’s computer systems in the United States and Europe were affected.

Who’s to blame?

Mr. Kurtz said CrowdStrike took responsibility for the software bug, sent in a system update, that caused the outage. He said in a post on X that Mac and Linux users were not affected.

The incident was not a cyberattack, Mr. Kurtz said, adding that customers remain “fully protected.” But Mr. Kurtz warned on NBC’s “Today” show that the fix could take some time to put in place.

“We understand the gravity of the situation and are deeply sorry for the inconvenience and disruption,” Mr. Kurtz said. Microsoft offered suggestions to users to help resolve the issue, including restoring backup systems.

While CrowdStrike is at fault for the software bug, J.J. Guy, chief executive of cybersecurity company Sevco, said poor resiliency of Microsoft’s operating system is to blame for extent of the damage.

“Bugs happen all the time and are unavoidable, the result of business complexity and technology,” Mr. Guy said. “But this became a catastrophic incident because of the remediation procedures. The resiliency of the operating system was not sufficient to mitigate the risk of that.”

Microsoft did not immediately respond to requests for comment on its operating system. The company’s chief executive, Satya Nadella, said in a post on X that Microsoft is working with CrowdStrike to offer customers technical guidance and bring systems back online.

Secret Service Director Kimberly Cheatle resigns after Donald Trump assassination attempt

WASHINGTON − Secret Service Director Kimberly Cheatle resigned Tuesday, 10 days after a lone gunman’s near-assassination of former President Donald Trump shook confidence in her ability to lead the agency charged with protecting presidents, candidates and other VIPs.

The agency confirmed later Tuesday that Ronald Rowe, the deputy director of the Secret Service, will take over as acting director.

Cheatle's departure comes just a day after she endured withering criticism by bipartisan lawmakers at a House Oversight Committee hearing Monday at which she admitted the July 13 incident was the biggest security failure in decades for the storied protective agency.

Numerous committee members − including top Republican James Comer of Kentucky and Democrat Jamie Raskin of Maryland − said she needed to step down and accused her of refusing to provide basic information about the attack and failed efforts by the Secret Service to stop the shooter, 20-year-old-Thomas Matthew Crooks, before he opened fire.

In a statement, President Joe Biden praised Cheatle, who protected him and first lady Jill Biden when Biden was vice president during the Obama administration.

"She has selflessly dedicated and risked her life to protect our nation throughout her career in the United States Secret Service. House Speaker Mike Johnson, R-La., told reporters Tuesday: "I'm glad to see that she has resigned. She should have done it sooner. Accountability begins at the top." He said the work of a congressional task force formed to investigate the attempt on Trump's life is much more important now that the head of the Secret Service has resigned.

Decision comes after brutal hearing, days of mounting pressure

Cheatle’s decision also came after days of mounting pressure from lawmakers and intense scrutiny over security failures at Trump's rally July 13 in Butler, Pennsylvania.

Trump was nicked in the ear after Crooks fired an AR-style assault rifle. The shooter had climbed up a nearby rooftop and fired off as many as eight shots, killing Pennsylvania firefighter Corey Comperatore and injuring two other men before being “neutralized” by a Secret Service countersniper, the agency said.

Trump came within a half-inch of suffering what could have been a fatal head shot wound. The GOP nominee himself said during his RNC acceptance speech Thursday that he wasn’t "supposed to be here " and that only a last-second turn of his head saved his life.

On Tuesday, Trump too criticized the Secret Service for what appeared to be the first time, saying on Truth Social that "The Biden/Harris Administration did not properly protect me, and I was forced to take a bullet for Democracy." Cheatle and other Secret Service officials have said repeatedly that politics do not play into their security efforts and that they protect all of their dignitaries to the best of their ability.

Cheatle's resignation was first reported by NBC News.

Biggest agency security failure in nearly half a century

Security experts have described the near-miss as the biggest security failure since President Ronald Reagan was nearly assassinated at close range in 1981. "A direct line of sight like that to the former president should not occur," Homeland Security Secretary Alejandro Mayorkas told "Good Morning America" two days after the shooting.

Biden and Mayorkas said an “independent review” would be launched into the Secret Service’s operational conduct. The Department of Homeland Security’s inspector general's office also announced it would investigate.

In his statement on Rowe's appointment as interim director, Mayorkas said, "I appreciate his willingness to lead the Secret Service at this incredibly challenging moment, as the agency works to get to the bottom of exactly what happened on July 13 and cooperate with ongoing investigations and Congressional oversight."

Pa. State Police also face questions over rally shooting

As the announcement of Cheatle's resignation was made Tuesday, Col. Christopher Paris, commissioner of the Pennsylvania State Police faced questions from the House Homeland Security Committee about the rally shooting.

Paris said the Secret Service was in charge of operations planning and that the Pennsylvania State Police were responsible for assisting the motorcade and staffing posts inside the secure area established by the Secret Service, which did not include the building from which the shooter fired some eight shots.

Paris also said he has not received a copy of the Secret Service’s operation plan despite his agency requesting it, which frustrated lawmakers. . Rep. Michael Guest, R-Mississippi, called the incident a “colossal failure” and said the Secret Service was “stonewalling” the state law enforcement agency by not providing it with the operation plan.

On Monday, a bipartisan group of lawmakers on the committee traveled to Butler, Pennsylvania, and toured the grounds where Trump spoke and the deadly shooting occurred.

Rep. Mark Green, R-Tennessee, said he was struck by how close the unsecured roof was from where Trump was delivering his speech. 

Green, the chair of the committee, said that “any excuse about the roof being too steep to post a counter sniper team up there is pure nonsense,” pointing to a statement made by Cheatle in a televised interview that the slope of the roof was the reason the building was secured only from the inside.

Cheatle's nearly six-hour tussle with lawmakers

Cheatle was subpoenaed to testify Monday morning about Secret Service failures before and after the attack, including how agents on the ground and atop a nearby building didn’t see – or stop – Crooks before he opened fire. Video shows numerous rallygoers pointing to him as he climbed the roof about 150 yards from where Trump was speaking. Her testimony, and the often-accusatory questions from lawmakers, lasted nearly six hours as many lawmakers expressed frustration about her inability or refusal to answer basic questions about the attack.

"I don't have the timeline of how the individual accessed the roof, where they accessed the roof, or how long they were on the roof," Cheatle acknowledged at one point.

“How did a 20-year-old loner with a week's notice pick the absolute best location to assassinate President Trump when the entire Secret Service missed it?” asked Rep. Pat Fallon, R-Texas. “Director Cheatle, on your leadership, your agency got outsmarted and outmaneuvered by a 20-year-old. How can we have any confidence that you could stop trained professionals from a nefarious nation state?”

Cheatle replied, “Those are absolutely questions that we need to have answered.”

A growing list of questions − and concerns

The investigation into the shooting, which is being led by the FBI, also found a significant lag time between the time Crooks was identified as a suspicious figure at the rally and when he was able to begin shooting.

Calls for Cheatle’s ouster, most of them from Republicans, grew louder after a closed-door briefing for lawmakers revealed additional troubling details about the Pennsylvania shooting.

Sen. John Barrasso, R-Wyo., posted on X , formerly Twitter, that "the head of the Secret Service needs to go." Senate Minority Leader Mitch McConnell, R-Ky., said new leadership would be an "important step" toward accountability.

On Friday, USA TODAY reported that former Secret Service officials also believe there were significant lapses in protocol after the shooting, when agents allowed Trump to stand and pump his fist three times, leaving him open to assassination by a potential second shooter.

Rep. Chip Roy, R-Texas, called for the Secret Service to be defunded, saying on Fox News that the shooting raised significant doubts about the agency’s ability to keep its protectees safe. Other lawmakers have questioned whether the agency did a good enough job planning security for the rally, especially after it was disclosed that local authorities said they were unable to guarantee security at the building from which Crooks took his shots after climbing up the side.

Some Republican lawmakers confronted Cheatle at the Republican National Convention in Milwaukee; video of them chasing her through the convention halls and demanding immediate answers about the security lapses – including why Trump was even allowed to take the stage – went viral.

The White House did not immediately comment or name a successor. The incoming leader will face the daunting task of restoring confidence in the agency and implementing sweeping security enhancements.

A former PepsiCo executive, trusted by the Bidens

Cheatle, the 27th director of the agency, was sworn in Sept. 17, 2022, according to her bio on the agency website. It said Cheatle was “responsible for successfully executing the agency’s integrated mission” of dignitary protection and investigations by 7,800 special agents and others.

Before her appointment, Cheatle was senior director in global security at PepsiCo, where she directed and implemented security protocols.

Before joining PepsiCo, Cheatle served “with distinction” for 27 years in the Secret Service, her bio said, including as assistant director of the Office of Protective Operations. She also was part of the security detail for then-Vice President Joe Biden and Jill Biden, forming a close bond with both of them.

“Jill and I know firsthand Kim’s commitment to her job and to the Secret Service’s people and mission,” Biden said when appointing her on Aug. 24, 2022. “When Kim served on my security detail when I was vice president, we came to trust her judgement and counsel. She is a distinguished law enforcement professional with exceptional leadership skills, and was easily the best choice to lead the agency at a critical moment for the Secret Service. She has my complete trust, and I look forward to working with her.”
