Field Trip Permission Slip Template

Our new permission slip template for field trips allows teachers to customize a letter and permission form to send home with students. It informs parents about the activity and has a cut-away section at the bottom with the emergency contact information and signature that the student will need to return to the teacher.

Description

This field trip permission slip includes an area at the top for you to explain more about the activity to parents and guardians. The two tables in the middle can be used to quickly provide the most important details about the activity like the location, date, time and what to bring.

What makes this template especially useful is the lower portion that can be cut off so that the student can bring back the signature and contact information of the parent or guardian. The parent can keep the upper portion as a reminder about the details of the activity.

We suggest finding a fun image to include as well. The text box in the template explains how to replace the existing image.

Our school has planned a field trip for 6th Grade students to Destination A on 12/12/2023. This field trip is intended to provide our students with a unique and educational experience that cannot be replicated in the classroom.

The Permission for Field Trips & Activities Slip is a form that must be filled out before you take any trip with any type of group of children whether it be church trip or any other kind of children's activities. The parents must read, fill out and sign the form before their child can participate in any field trip or any youth activity. In some cases, depending upon your church, they might even have you notarize the form before returning it to the church office but of course this would be up to your specific church and their policies.

The Permission for Youth to Volunteer in the Children's Ministry form is what the parent has to read, fill out and sign before their child can volunteer to help as a Youth Worker Volunteer in any type of children's activity. They would be able to participate in such church activities such as assisting in registration, nursery, classrooms, special church events and much more with the supervision of the Children's Ministry adult volunteers.

Field trips are very exciting and fun events for the children to enjoy and make memories. The church can make this a fun and enjoyable event for the children and at the same time make sure every child is covered by having the correct paperwork in place so there will not be any problem arise that might hinder the church from making sure the child is properly taken care of in the unexpected event of an emergency, etc.

When the Children's Ministry at your church begins the long process of planning an all day or extended field trip, there are many very important details that have to be addressed first such as the availability of the church buses, cars or vans before you make reservations or purchase tickets for your field trip with the children so be sure to do your preplanning ahead of time to prevent any delays or cancellations occurring and disappointing a lot of children.
